Experience advanced facial rejuvenation with a deep plane facelift, a surgical procedure that restores youthful structure, smooth deep folds and redefines the cheeks, jawline and neck for natural, long-lasting results.
If you’re looking to lift sagging facial tissues, smooth deep folds and restore youthful definition to your face and neck, a deep plane facelift may be the ideal option. This advanced surgical procedure works beneath the surface of the skin to reposition the deeper facial structures that naturally descend with age, creating a natural-looking result that lasts.
At Paul Wilson Aesthetics, our procedures are performed by Mr Paul Wilson who will support you at every stage, from initial consultation through to surgery and recovery.
About
A deep plane facelift is an advanced facial rejuvenation procedure that lifts and repositions the deeper support layers of the face, rather than simply tightening the skin. By working beneath the SMAS layer (the facial muscle and support structure), this technique allows the surgeon to redefine the jawline, smooth out platysmal bands and naturally lift the midface, jowls and lower face, restoring youthful contours with long-lasting, natural-looking results.
Unlike traditional facelifts, which mainly address surface laxity, a deep plane facelift corrects the underlying cause of sagging for a smoother, more refreshed appearance without a tight or over-pulled look.
Types
A deep plane facelift can be tailored to target specific areas of the face, depending on where ageing changes are most noticeable. Your surgeon will recommend the most appropriate approach based on your anatomy, skin quality and goals.
This technique focuses on the central portion of the face, particularly the cheeks and the area beneath the eyes. It is designed to lift sagging cheek tissue, soften deep folds around the nose and mouth and restore youthful cheek contours. A deep plane mid-face lift is ideal for patients who notice heaviness or flattening in the upper cheeks but do not yet require a full facial lift.
A deep plane lower face lift targets the lower cheeks, jowls and jawline. It is particularly effective for correcting sagging along the jawline, softening jowls and restoring a sharper, more youthful lower facial contour. This approach is often chosen by patients whose main concern is loss of definition in the lower face.
A full deep plane facelift combines both mid-face and lower face correction for comprehensive facial rejuvenation. It repositions the deeper tissues across the cheeks, jowls and jawline, creating balanced, natural-looking results across the entire face.
Combinations
Facial fat grafting is often performed alongside a deep plane facelift, as the two procedures address the two primary signs of facial ageing:
When performed together, these treatments work synergistically to rejuvenate the face. The facelift restores structure and lift, while fat grafting replenishes lost volume in areas such as the cheeks, temples, under-eyes and around the mouth.

candidates
While many patients seek a deep plane facelift in their 40s and beyond, the right timing varies based on genetics, lifestyle and personal goals. A consultation with your surgeon will determine whether this procedure is appropriate for you.
A deep plane facelift may be suitable for individuals who:
What to expect
During your consultation, your surgeon will conduct a thorough assessment of your face and neck, identifying areas of sagging, volume loss and changes in skin quality. They will review your medical history, discuss your aesthetic goals and explain how a deep plane facelift can reposition the deeper facial tissues for a natural, long-lasting lift.
Your surgeon will also outline what to expect during recovery and will provide personalised advice on how to prepare for surgery. Typical incision placement will be discussed, and photographs may be taken to assist with planning and to help guide the surgical approach.
This appointment is your opportunity to ask questions, explore your treatment options and feel fully informed and confident before proceeding.
A deep plane facelift is performed under general anaesthesia. When carried out on its own, the procedure typically takes around three to four hours, depending on the extent of lifting required.
When facial fat grafting is combined with a deep plane facelift, the overall procedure time is usually four to six hours, as both the deep structural lifting of the face and the precise placement of grafted fat are completed in the same operation. Your surgeon will confirm the expected duration of your procedure during your consultation, based on your individual treatment plan.
During the procedure, the surgeon works beneath the deeper support layers of the face, gently releasing the ligaments that contribute to sagging. Both the skin and the deeper SMAS layer are lifted together as a single unit, allowing the cheeks, jowls and neck to be repositioned in a natural upward direction. This approach creates a more youthful contour without the tight or “pulled” appearance associated with traditional facelifts.
For a deep plane facelift, incisions are carefully placed so they are as discreet as possible, typically within the hairline, around the front of the ear and behind the ear.
When facial fat grafting is performed alongside the deep plane facelift, a small volume of fat is gently harvested from donor areas such as the abdomen, flanks, or thighs using fine cannulas.
The fat is then carefully purified and injected into targeted facial areas such as the temples, cheeks, under-eye region and around the mouth. This restores volume, softens contours and improves overall skin quality.
Once lifting (and any facial fat grafting) is complete, the facial tissues are carefully re-draped without tension, and the incisions are closed using fine sutures. Temporary drains may be placed to help prevent fluid build-up, and light compression dressings are applied to support healing. In some cases, a haemostatic net may also be used to minimise swelling and reduce the risk of bruising or haematoma.
Recovery
Recovery varies from person to person and depends on the extent of surgery and whether facial fat grafting has also been performed at the same time.
Bruising and swelling are most noticeable in the first few days after surgery and usually peak around day three to four. These effects gradually improve over the following two to three weeks. As swelling reduces, the lifted contours of the face and neck become increasingly visible.
Light walking is encouraged soon after surgery to support circulation and reduce the risk of complications. Strenuous exercise, heavy lifting and activities that significantly raise your heart rate should be avoided for at least two weeks, or as advised by your surgeon.
Follow-up appointments will be scheduled to monitor your healing and remove sutures if required. The face continues to refine over several months as swelling fully settles and the deeper tissues stabilise. Final results are typically visible between three and six months and are long-lasting, particularly when supported by a healthy lifestyle.
Risks & complications
Your surgeon will discuss all potential risks with you during your consultation to ensure you are fully informed before making any decisions.
As with any surgical procedure, a deep plane facelift carries certain risks. These may include:
The cost of a deep plane facelift depends on the complexity of your surgery and whether additional procedures, such as facial fat grafting, are performed at the same time. A personalised quote is provided following your consultation.
While a deep plane facelift may have a higher upfront cost than some other treatments, its long-lasting and comprehensive results often mean fewer future procedures. For many patients, this makes it a valuable long-term investment.
FAQ
A deep plane facelift lifts the deeper facial support layers rather than just tightening the skin. This allows the face to be repositioned more naturally, avoiding the tight or stretched look that can occur with traditional facelifts. The result is a softer, more youthful and longer-lasting outcome.
Initial recovery usually takes 2 to 3 weeks, during which swelling and bruising gradually settle. Many patients return to light daily activities after around two weeks. The face continues to refine over several months as deeper tissues fully heal.
Results typically last 10 to 15 years or longer. While ageing continues naturally, the deeper structural lift achieved with a deep plane facelift allows the improvements to remain visible for many years.
A deep plane facelift is carried out under general anaesthesia. Carefully placed incisions allow the surgeon to lift and reposition the deeper tissues of the face before re-draping the skin without tension.
When performed on its own, the procedure usually takes three to four hours. If combined with facial fat grafting or additional lifting procedures, surgery may take longer.
The deep plane facelift uses carefully placed incisions that follow natural contours of the face and ears, including within the hairline, in front of the ear and behind the ear. Once healed, these scars are designed to be discreet and typically blend well into the natural skin creases and hairline, making them difficult to notice.

Gynaecomastia | Before & After
This procedure addresses excess fullness in the male chest caused by gland, fat, or a combination of both.
The aim is not simply to remove tissue, but to improve the overall chest shape for a balanced and natural appearance. As with any operation, success depends on choosing the right technique for the individual`s anatomy and careful surgical planning from the outset.
The initial result is just one stage of the process. Swelling gradually subsides, and the chest continues to refine as healing progresses.

A neck lift is designed to address laxity beneath the chin and along the jawline.
Some patients are less concerned with their face itself, and more with the way their neck has changed over time. Loss of definition, heavier tissue, or loose skin in this area can affect overall facial balance.
A neck lift focuses on restoring a more defined contour through careful surgical planning, with the aim of achieving a result that looks natural for the individual.
As with any procedure, suitability depends on anatomy, skin quality, and the changes being treated.

Augmentation Mastopexy | Before & After
Her primary concern was not size, but rather the degree of ptosis (drooping) and the altered shape of her breasts.
This distinction is important, as it fundamentally changes the surgical approach.
In this instance, the breasts required lifting and reshaping, not merely augmentation. We discussed achievable outcomes, the degree of support the existing tissues could provide, and the appropriate implant volume to achieve optimal results once the breasts were repositioned. The procedure combined a mastopexy with implants to address these issues concurrently, with the aim of creating a more balanced overall appearance.
These procedures require a meticulous approach. The patient`s initial anatomy, tissue quality, and the extent of lift required all influence the surgical plan.
The result shown here is the outcome of that process, although healing continues well beyond the initial stages.

Abdominoplasty | Before & After
The abdomen can be one of the most challenging areas to restore after the tissues have been stretched. Even in individuals who are otherwise fit and healthy, excess skin and reduced support can persist.
In a suitable patient, abdominoplasty can improve the abdomen`s shape by removing excess tissue and addressing the underlying support where necessary. The aim is not to create an artificial appearance, but to restore a contour that feels more proportionate to the rest of the body.
As with any surgery, healing is an integral part of the process. Swelling subsides over time, scars soften gradually, and the final result continues to improve.

INDIRECT LIP LIFT + TRANSNASAL ALAR CINCH — REVISION CASE ✨
Top: before. Bottom: after revision.
This patient presented after a previous lip procedure left him with an excessively long upper lip at 18mm, minimal vermillion show, and a wide alar base disrupting central facial harmony.
Post-revision: philtral length reduced to 12mm — a 6mm correction into ideal proportion (12–15mm is the youthful range for men) — paired with a refined, narrower nasal base.
Two procedures, one harmonious result:
🔸 Indirect Lip Lift (Subnasal Bullhorn) — precise excision of skin beneath the nose to shorten the upper lip, increase pink lip show, and restore the cupid’s bow. No filler.
🔸 Transnasal Alar Cinching Suture — a rhinoplasty technique specialised by Mr Wilson. A hidden suture passed through the nose draws the alae medially, narrowing flared nostrils with no visible external scarring — a major advantage over traditional alar base reduction.
Revision work demands exact measurements, respect for existing scar tissue, and meticulous management of altered tension vectors. There is no margin for over-correction.
The synergy is what creates harmony: a shorter lip without a refined base looks disproportionate; a narrowed base alone leaves the lower face heavy. Together — balanced, masculine, natural.

✨ Revision Rhinoplasty at Paul Wilson Aesthetics ✨
Precision. Expertise. Artistry.
Revision rhinoplasty is one of the most technically demanding procedures in facial plastic surgery — and it’s a cornerstone of our practice at Northwood Hospital. Patients travel to us from across the country because secondary nasal surgery requires a level of skill, planning, and experience that few surgeons offer.
Every revision case is unique. Scar tissue, altered anatomy, and previous structural changes mean there’s no room for a one-size-fits-all approach. We rebuild with care — restoring both function and the natural, balanced result patients hoped for the first time.

Welcome to Paul Wilson Aesthetics — where surgical precision meets artistry. Led by Mr Paul Wilson, our clinic specialises in advanced facial and aesthetic procedures designed to enhance your natural features and restore facial harmony. With years of expertise and a meticulous, patient-first approach, we’re proud to be a trusted name for those seeking refined, natural-looking results.
All surgical procedures are carried out at Northwood Hospital, Bristol 🏥 — a private hospital known for its exceptional standards of care, comfort, and clinical excellence.
One of our signature procedures? Hairline lowering surgery (foreheadplasty).
A high or elongated forehead can throw off the natural proportions of the face — and for many of our patients, it’s something they’ve felt self-conscious about for years. Hairline lowering brings the hairline forward in a single procedure, instantly restoring balance to the upper third of the face.
What it can do:
🌿 Reduce forehead height by up to 2.5cm in one procedure
🌿 Create softer, more feminine facial proportions
🌿 Frame the face beautifully
🌿 Deliver immediate, natural-looking results
Every journey at Paul Wilson Aesthetics begins with precise measurements and a personalised consultation — because no two faces are the same, and careful planning is what turns a good result into a stunning one.

Sometimes, the first signs of ageing appear around the eyes.
Excess skin or heaviness in the upper eyelids, or puffiness beneath the eyes, can make the face look more tired than it feels. Blepharoplasty is designed to address these changes with a careful, tailored approach to the upper eyelids, lower eyelids, or both.
The aim is not to change the eyes` appearance, but to restore a fresher, more open look while ensuring the result is natural and in harmony with the rest of the face.
As always, the most suitable surgical plan depends on the individual, their anatomy, and the underlying cause of their concerns.

Foreheadplasty | Before & After
This type of surgery is less about changing a feature and more about restoring balance.
Where the forehead feels too dominant, lowering the hairline can alter the proportions of the upper face in a meaningful way. The procedure, often referred to as forehead reduction or hairline lowering, advances the scalp to reduce forehead height, with the plan carefully tailored to the individual.
As always, what matters most is choosing the right operation for the right reason, then allowing time for healing and scar maturation afterwards.

Augmentation Mastopexy | Before & After
Over time, she felt her breasts had lost fullness, shape, and position. Her main concern was not simply size, but the way her breasts sat and the lack of support in the upper part of the breast.
We discussed the changes she had noticed and the type of result that would feel balanced for her figure. The plan was to restore upper fullness and improve breast position in a way that looked proportionate and in keeping with her frame. To achieve this, I combined a breast lift with implants, allowing both shape and volume to be addressed in the same operation.
Augmentation mastopexy is a procedure that requires precision and judgement. The tissues need to be managed carefully, and the surgical plan must be tailored to the individual anatomy, rather than following a standard formula.
The early result is only part of the process. The breasts will continue to settle, soften, and refine as they heal over time.

Abdominoplasty | Before & After
Changes to the abdomen can involve multiple concerns simultaneously. Excess skin, reduced abdominal wall support, and an altered overall contour can all affect how this area sits in relation to the rest of the body.
In this case, the aim of surgery was to improve the abdominal contour in a way that looked balanced and suited the individual. As always, this begins with a careful assessment, a clear plan, and an honest discussion about what surgery can realistically achieve.
Healing takes time. Swelling settles gradually, scars continue to mature, and the result refines over the following months.
