Hairline-lowering surgery, also known as forehead reduction surgery or foreheadplasty, is a procedure that reduces the height of the forehead by lowering the hairline. It is commonly performed to address aesthetic concerns related to a high or receding hairline caused by genetics, ageing or hair loss.
Mr Paul Wilson performs all hairline-lowering surgeries at Northwood Hospital. He employs meticulous techniques to ensure minimal scarring and optimal symmetry, tailoring each procedure to the unique needs of each individual.
About
A higher hairline can develop for a variety of reasons, including genetic traits, age-related changes or previous hair thinning. For some individuals, this can create the appearance of a longer or less balanced forehead, which may affect how the upper face relates to the rest of the facial features.
Hairline-lowering surgery is an option for those seeking to improve facial balance and proportion. By carefully repositioning the hairline, the procedure aims to achieve a result that looks both natural and aesthetically pleasing.

What to expect
Your consultation is an important first step in determining whether hairline-lowering surgery is suitable for you. During this appointment, Mr Wilson will carefully assess your facial structure, hairline position, scalp laxity and hair density. He will discuss your goals and concerns and review your medical history to determine the most appropriate treatment approach.
Mr Wilson will also explain the procedure in detail, outline what to expect during recovery and answer any questions you may have. This consultation is your opportunity to ensure you feel fully informed before proceeding.
Hairline-lowering surgery is typically performed under general anaesthesia to ensure patient comfort throughout the procedure. The surgery usually takes around two hours, depending on individual anatomy and the extent of adjustment required. Mr Wilson will confirm the expected procedure time with you during your consultation.
Hairline-lowering surgery begins with careful planning and marking of the new hairline position. Then, a strategically placed incision is made along the natural hairline to blend discreetly into the surrounding hair and minimise the visibility of scarring.
During the procedure, the scalp is gently advanced to the newly marked position and excess skin is removed to shorten the vertical length of the forehead. The repositioned scalp is then secured using an Endochor device (carpet gripper) to prevent any tension and support long-term stability.
Special care is taken throughout the procedure to preserve hair follicles and maintain healthy hair growth. Once the adjustment is complete, the incision is closed using fine sutures to promote optimal healing and minimise scarring.
Recovery
Recovery following hairline-lowering surgery varies between individuals.
After surgery, it is normal to experience mild swelling, bruising and some discomfort around the incision area. These effects are usually most noticeable during the first few days and typically settle within one to two weeks. A headband or light compression may be recommended during the early recovery period to support healing.
The incision line may initially appear pink and gradually fades over time, blending naturally with the surrounding hair. Many patients notice an improvement in facial proportions soon after surgery, with results continuing to refine as swelling subsides.
Most patients can return to light daily activities within a week. Returning to work is typically possible within 7 to 10 days, depending on individual recovery and the nature of your job. Strenuous exercise and heavy physical activity should be avoided until advised by your surgeon.
Follow-up appointments will be scheduled to monitor healing and ensure recovery is progressing as expected. Final results become more apparent as swelling fully resolves, with outcomes continuing to improve over the following weeks and months.
Risks & complications
As with any surgical procedure, hairline-lowering surgery carries some risks and potential complications. These may include:
Your surgeon will discuss all potential risks during your consultation, ensuring you are fully informed before deciding to proceed.
Fees
The cost of hairline-lowering surgery depends on the complexity of the procedure and individual factors such as scalp laxity and the extent of adjustment required. These factors can influence the overall cost of treatment.
A personalised quote will be provided following your consultation. During this appointment, your surgeon will explain all pricing details clearly, ensuring you have a full understanding of costs before proceeding.

FAQ
Hairline-lowering surgery typically takes 1.5 to 2 hours, depending on the complexity and the amount of adjustment required. This includes preparation, the actual procedure and initial post-operative care. Mr Wilson ensures a meticulous approach, tailoring each surgery to achieve the best results. You will be given detailed information on your expected surgical time during your consultation.
Hairline-lowering surgery is considered a safe and effective procedure when performed by an experienced surgeon like Mr Wilson. Potential risks are minimised through advanced surgical techniques and meticulous pre- and post-operative care.
After surgery, patients must keep the incision area clean and follow Mr Wilson’s instructions for wound care. Avoid strenuous activities for 1-2 weeks and protect the area from direct sun exposure. A headband or light compression may be recommended to support healing.
Your first follow-up appointment will typically occur 10 days after surgery to check on healing and remove sutures. Additional follow-ups may be scheduled over the following months to monitor progress and ensure satisfaction with results.
Hairline-lowering surgery provides permanent results. Once the scalp is repositioned, it remains in place. While normal ageing may alter the appearance slightly over time, most patients do not require repeat procedures. Mr Wilson designs each surgery to deliver long-lasting, natural results.
Yes, you can have a hair transplant after hairline lowering surgery. In fact, it’s often used to refine the hairline and blend or disguise the surgical scar. If any areas of thinning or alopecia develop following the procedure, a hair transplant can help restore density and create a more natural-looking result.
Yes, it is possible to combine a brow lift with forehead-lowering surgery. In fact, performing both procedures together can create one of the most powerful and effective brow-lifting results, as they can be completed through the same access incision. Additional Endotine (or similar) devices may also be used to fine-tune the shape and position of the brows.
Yes, you can undergo a hairline lowering procedure even if you’ve previously had a hair transplant. In some cases, it can further enhance your original transplant results or help revise an area of grafting that no longer meets your aesthetic goals. Your surgeon will assess your hairline, density and scalp laxity to determine the safest and most effective approach.
Yes, you can still have a hairline lowering procedure after a brow lift. Brow lifts often elevate the forehead and brow position, which can increase forehead height. If hair-bearing tissue was removed during your previous surgery or your forehead now sits higher than desired (typically more than 6 cm), a foreheadplasty can be performed to bring the anterior hairline lower and restore balance to your features.
There is no single “ideal” forehead height, as facial beauty is highly individual and influenced by personal preference and overall facial harmony. However, when considering classical proportions, many patients find a forehead height of around 5.5–6.0 cm—measured from the glabella (the point between the eyebrows) to the anterior hairline—to be aesthetically balanced. Ultimately, the most suitable height depends on your unique features and goals.
Most forehead reduction procedures can safely lower the hairline by around 2.0–2.5 cm. In select cases with excellent scalp laxity, larger advancements are possible, the greatest reduction that Mr. Wilson has ever done, for instance, is 4.0 cm.

Restoring shape, volume, and confidence.
Augmentation Mastopexy (often called a breast uplift with implants) is a dual-benefit procedure designed for patients who have lost breast volume—often due to weight loss, pregnancy, or breastfeeding—and require both lifting and refilling.
The Goal: Many patients come to me feeling their breast tissue has become “empty” or sits lower on the chest wall. A standard implant alone often isn’t enough to correct significant drooping (ptosis), and an uplift alone may not provide the upper-pole fullness many desire. By combining these techniques, we can: 1️⃣ Remove excess skin to lift the breast position. 2️⃣ Insert an implant to restore lost volume and firmness.
The Result: As seen in these before and afters, the outcome is a rejuvenated, natural silhouette that looks proportionate to the patient’s frame.
We plan every surgery with precision using Vectra 3D imaging during your consultation, allowing us to visualize the potential outcome together before you ever step into theatre.

🔹 Abdominoplasty – Functional and Reconstructive Benefits 🔹
This case highlights the functional benefits of abdominoplasty beyond aesthetic improvement. Abdominoplasty can significantly enhance core stability by repairing rectus diastasis, improve posture, and alleviate lower back discomfort associated with weakened abdominal musculature.
Patients may also experience improved mobility and tolerance for physical activity following reduction of excess abdominal tissue, which can lead to enhanced overall quality of life.
I am very grateful to this patient for kindly allowing me to share her clinical photographs. Her case exemplifies the reconstructive potential of abdominoplasty in the appropriate clinical context.
